Referring Physician Access Request

Our Telerad partners may request that their referring physicians be considered for access to our portal by submitting the following information. Please note that submitting a request does not guarantee access privileges. A member of our team will communicate details if approval is granted.

NOTE: Requests must be submitted by our Telerad partner organizations on behalf of their referrers; direct requests from referring physicians or their respective practices will not be accepted.
